After I read the questionaires, I think there is some shortages such as what their interests are, what material do they like, when they use English(situation).

most of the questions on this questionaire is good ... i could analyze the purpose of you to write the questions like this,, but i think there is something that i don't understand that is the question about "which teacher do you like"?? what is the purpose of this question>>>.??? may be it is better to write like this.. what is the characteristic of the teacher that you wish will teach you english later on the course??? so,, by answering this question,, we could know what will we be and how will we act in the classroom on learning process later......
that's my little suggestion anyway
